Billing & Fee Policies


BILLING STATEMENT FORMAT:

Our billing statements indicate the amount(s) any insurance or vision plan will not be paying. We ask for your understanding with this format. Until any insurance pays there will be a balance owed on the statement. However, the patient's amount to pay out of the balance is notated.

TIME OF PAYMENT:

Unless prior arrangements are made or the vision plan is being billed, payment for services and/ or products is expected when received. A 50% deposit is needed to order eyeglasses with the remainder due upon dispensing. Discounts are available for those who pay in full up front for their eyeglass order.

YOUR INSURANCE COVERAGE AND OUR OFFICE POLICIES:

Your vision plan is a relationship between you and your insurance company. Please be aware that some or all of the services we provide may not be considered necessary by your insurance company. Your insurance company will determine which benefits are eligible after your claim has been submitted. Any co-payment collected from you at the time of service is only an estimate of your part. There are certain insurance companies we participate with. We participate with VSP, Medicare, Blue Cross and Blue Shield (Vision and Medical), Eyemed (Eyecare Plan of America), Superior Vision Service, Connect Care, Cigna, Aetna. Call if yours is not listed, and we accept their payment in full, excluding any copayments or deductibles indicated by your insurance company.

CONTACT LENS FEES:

Eye examinations for contact lens wear requires Dr. Buckingham or Dr. Kaminski to perform additional measurements and testing thus the exam fee is higher than an examination for glasses. When new patients do not know what contact lens brand they are wearing or if any patient requires refitting into a new contact lens brand, the doctors charge a refitting fee based on the complexity of your contact lens refit. Included in the refitting is the time required, follow up contact lens checks, trial contact lenses for disposable contacts, and for rigid gas permeable lenses, a limited warranty period. Feel free to ask the staff for an estimate for the costs of your contact lens refitting.

OPTICAL WARRANTIES, REPAIRS, AND ADJUSTMENTS:

Our office provides at no extra cost a one year warranty on all frames purchased. This warranty is limited to manufacturer defects or breakage from the intended use of eyeglasses. We are happy to perform complimentary small repairs and adjustments on your eyeglasses, even if they were not purchased at our office. We strive to make your eyewear function and last as long as possible.
